Care Manager RN - Care Management at Providence St. Vincent Medical Center. This is a Full-Time (1.0 FTE), Day Shift position, with five (5) 8-Hour Shifts from 800-1630.
RN Care Managers, in the acute care setting, provide a variety of services to appropriately help progress patients through the continuum of care. Key services include screening for post-acute needs; transition planning; care coordination; education; community care referrals; care planning in accordance with regulatory requirements; collaboration with the continuum care team; ongoing monitoring of plan effectiveness and reassessment of the need for Care Management (CM) services.

Required Qualifications:
- Nursing degree/diploma required upon hire.
- Bachelor's degree in Nursing or higher within 3 years of hire.
- Oregon Registered Nurse License upon hire.
- 2 years acute care hospital or community based experience.
- 1 year experience with electronic medical records or documentation systems.
- 1 year Home health, mental health, substance use, hospice, utilization review, discharge planning or care management experience.
Preferred Qualifications:
- Bachelor's degree in nursing upon hire.
- Upon hire: Certified Case Manager.
