About the Position

Sayf Primary Care is seeking two experienced, highly professional Clinical Medical Assistants and Care Coordinators to join our close-knit primary care team.

The roles focus on direct patient care, clinical support, chronic care management, and care coordination. These positions are ideal for nursing or advanced education students.

We are looking for people who are naturally upbeat, warm, outgoing, and comfortable building relationships with patients. The right candidates can carry thoughtful conversations, make patients feel genuinely cared for, and communicate with professionalism and emotional maturity—even when assisting patients with complex medical or social needs.

These are phone-heavy and patient-facing positions requiring excellent follow-through, sound judgment, accurate documentation, and the ability to work closely with our physicians and nurse practitioners. Prior experience using eClinicalWorks (ECW) is required.

Primary ResponsibilitiesDirect Patient Care

• Room patients and obtain complete, accurate vital signs
• Review and document medical histories, medications, allergies, and current concerns
• Perform excellent venipuncture on patients with varying levels of difficulty
• Administer injections, vaccines, and medications accurately and confidently
• Perform EKGs, point-of-care testing, and other clinical procedures
• Prepare specimens, maintain laboratory logs, and coordinate specimen processing and pickup
• Provide patient education and reinforce provider instructions
• Maintain clean, organized, and properly stocked clinical areas
• Follow infection-control, patient-safety, and clinical documentation standards

Chronic Care Management and Patient Follow-Up

• Conduct scheduled telephone check-ins with chronic care management patients
• Build comfortable, professional relationships with patients through regular communication
• Ask appropriate questions regarding symptoms, medications, treatment adherence, recent appointments, and changes in health
• Identify and clearly document patient concerns for provider review
• Provide concise, useful assessments and updates to physicians and nurse practitioners
• Follow up with higher-risk or medically complex patients, including weekly outreach when directed
• Reinforce care plans and help patients understand their next steps
• Escalate urgent or clinically significant concerns promptly to the appropriate provider
• Document all outreach, patient responses, and care-coordination activity thoroughly in ECW

Clinical Coordination and Provider Support

• Process medication refill requests accurately and route them to the appropriate provider
• Prepare and coordinate laboratory, diagnostic, imaging, and specialist orders
• Initiate and follow through on referrals, ensuring that necessary records and orders reach the appropriate provider
• Obtain required prior authorizations directly from insurance companies for procedures, diagnostic testing, imaging, medications, and other ordered services, and follow each request through determination
• Communicate with specialists, pharmacies, laboratories, insurance companies, home-health agencies, facilities, and other healthcare professionals
• Track outstanding orders, referrals, prior authorizations, test results, and patient-care needs through completion
• Monitor and respond to clinical telephone calls, ECW tasks, and Healow messages
• Assist with medication reconciliation and resolution of medication-related questions
• Support preventive-care and chronic-disease workflows, including screenings, immunizations, diabetic care gaps, and transition-of-care follow-up
• Work closely with physicians and nurse practitioners to ensure that nothing affecting a patient’s care is overlooked

On-Site Facility Care

• Accompany and support the provider during scheduled visits to a local independent-living facility
• Prepare patients, obtain vital signs, update medication and health information, and assist with clinical procedures
• Coordinate follow-up care, orders, referrals, prior authorizations, and communication resulting from facility visits
• Maintain the same level of professionalism, warmth, and clinical organization in both the office and facility settings

Required Qualifications

• Prior professional Medical Assistant experience
• Prior hands-on experience using eClinicalWorks (ECW)
• Medical Assistant certification or equivalent formal training and clinical experience
• Excellent venipuncture skills; candidates must be confident drawing patients with both routine and difficult access
• Strong, demonstrated ability to administer injections and vaccines safely
• Excellent ability to obtain and document complete, accurate vital signs
• Experience performing EKGs accurately
• Experience performing and documenting point-of-care testing
• Strong verbal and written communication skills
• Professional telephone presence and comfort conducting frequent patient outreach
• Ability to write clear, clinically useful notes and patient-status updates
• Strong organizational skills and attention to detail
• Ability to prioritize responsibilities and consistently follow tasks through to completion
• Emotional maturity, good judgment, discretion, and respect for patient confidentiality
• Ability to interact professionally with patients, families, providers, pharmacies, insurance companies, facilities, and outside healthcare organizations
• Dependable transportation and ability to work at both our Oakbrook Terrace office and a local independent-living facility
• Flexibility to work variable part-time hours based on patient-care and practice needs

Preferred Qualifications

• IV placement or IV therapy experience
• Spotfire experience
• Experience performing respiratory testing
• Experience with chronic care management
• Experience supporting medically complex, geriatric, or high-risk patients
• Experience with referrals, prior authorizations, medication refills, and care-gap tracking
• Experience in primary care or internal medicine

We are specifically seeking experienced professionals who can combine excellent hands-on clinical skills with exceptional communication and patient-care abilities. Schedule and Location

• Two part-time, in-person positions are available
• Hours and days may vary based on practice and patient-care needs

• Regular work at a local independent-living facility is part of these positions

Compensation

• Starting compensation is $23 per hour during the initial 90-day evaluation period.
• At the completion of 90 days, performance and compensation will be reassessed. Any pay adjustment will be based on demonstrated performance, clinical competency, reliability, professionalism, and success in fulfilling the responsibilities of the position.
