Position: Registered Nurse / RN Team Director

BASIC QUALIFICATIONS
- Selected candidate MUST have 5 yrs RN experience and at least 2 years as a Nursing Supervisor.
- ADN or higher required
- Five or more years of experience as a registered nurse required
- Minimum of two years of healthcare supervisory experience required
- Certified Hospice Nurse (CHPN) preferred
- Bilingual English/Spanish preferred
- Three+ years' experience in hospice, home health, general medical surgery, oncology, or patient population-specific nursing (i.e., geriatrics, pediatric, etc.)
- Possess and maintain valid CPR certification while employed in a clinical role (preferred)
- Must be a licensed driver who can travel to all business locations
- Meet the regulations and requirements of the state(s) in which the program provides services
YOUR ROLE AND RESPONSIBILITIES
- Supervise direct patient and family care team members, in collaboration with program site leadership
- Responsible for scheduling, payroll, and other administrative duties
- Ensure all initial assessments and required reassessments are conducted by qualified team members within required time frames, including documentation submissions, and/or synchronization
- Oversee and support the Interdisciplinary Group (IDG) in development, implementation, and review of the interdisciplinary plan of care for each patient and family
- Collaborate with program site leadership members to mentor, manage and monitor staff in patient care activities, productivity, and documentation, adhering to related policies and protocols
- Participate in and promote Back-to-Bedside efforts in reinforcing Case Management skills, and knowledge and attitude per protocol for all disciplines
- Collaborate with site leadership to support staff education efforts
- Facilitate IDG meetings and provide ongoing IDG Support such as triaging incoming calls, providing caller support and assistance, and performing quality calls
- Participate in or lead quality improvement activities, including data collection and aggregation
- Reinforce and support all internal and external communications
