As a Home Health Physical Therapist at AccentCare, you'll bring skilled, compassionate care directly to patients’ homes. You'll assess mobility needs, create personalized treatment plans, and guide exercises that restore strength, balance, and independence. You'll provide hands-on support after illness, injury, or surgery while educating patients and families. Through clinical expertise and genuine connection, you'll help people reclaim everyday moments that matter!
Physical Therapist Key Responsibilities
- Conduct thorough patient evaluations: Assess mobility, functional limitations, safety risks, and therapy needs to establish a clear roadmap for successful in-home rehabilitation.
- Create personalized treatment plans: Design evidence based, goal driven therapy plans tailored to each patient’s condition, supporting meaningful gains in strength, independence, and confidence.
- Deliver skilled, hands-on care: Provide one-on-one therapeutic interventions that improve mobility, balance, and functional ability—directly shaping patient outcomes and quality of life.
- Educate and empower patients and families: Teach home exercise programs, safety techniques, and ongoing care strategies that encourage progress between visits and help prevent physical therapy setbacks.
- Monitor progress and communicate changes: Track physical therapy results, adjust treatment plans, and collaborate with physicians and the care team to ensure safe, effective, and timely care.
Physical Therapist Qualifications
- Community and/or home health experience preferred
- Master’s degree in physical therapy approved by a CAPTE-accredited physical therapist education program
- Current physical therapist license within the state of agency operation, and compliant with accepted professional standards and practices
- Valid CPR certification required
- Must be a licensed driver who can travel to all business locations
