Home visits in assigned service area: Largo, St. Petersburg, Pinellas Park surrounding areas assigned by zip code.
The Home Health Therapy Lead I provides hands-on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols. Responsibilities include:
- Eliminates barriers to care improving access to appropriate post-acute care rehabilitation, home, and community resources.
- Provides planning functions to assure the best continuity of care efficiently and effectively, and efficiently guide assigned patients and other related stakeholders through the post-acute care rehabilitation process under the supervision of the Rehab Services Manager.
- Serves as a single point of contact for referring physicians. Coordinates, provides resources and assistance for less than 499 patients per year and caregivers when accessing home care and supportive care services.
- Provides direct patient care.
- Job Specific Functions also includes: Coordinates less than 499 patients per year for payer types: Medicare or Managed Care or other insurances; Intensity of lead therapy interactions equals 4 hours or less per week for low to medium physician's office maintenance.
Home Health Therapy Lead I Requirements:
- Bachelor's Physical Therapy
- BLS (Basic Life Support)
- FL PT (Physical Therapy); Or – OT (Occupational Therapy) license
- FL Driver's License (required within 30 days of hire)
- 1 Year Physical Therapist; Or - 1 year Occupational Therapy experience
