Position Summary
We are seeking a dedicated and board-certified (or board-eligible) Family Medicine Physician to join our growing primary care team in Far Rockaway, NY. The ideal candidate will provide high-quality, evidence-based medical care across the lifespan—from pediatrics to geriatrics—while emphasizing preventive health and chronic disease management.
Key Responsibilities
- Provide comprehensive outpatient primary care for patients of all ages.
- Diagnose and manage acute and chronic medical conditions, including hypertension, diabetes, asthma, and heart disease.
- Conduct preventive health screenings and wellness visits.
- Deliver patient education on diet, exercise, and behavioral health.
- Perform minor office-based procedures as appropriate (e.g., suturing, skin lesion removal, joint injections).
- Coordinate care with specialists and ancillary providers to ensure continuity and quality of care.
- Maintain accurate and timely medical records in accordance with clinical and regulatory standards.
- Participate in quality improvement initiatives and population health programs.
- Engage in health promotion and community education efforts.
Qualifications
- MD or DO degree from an accredited medical school.
- Board Certified or Board Eligible in Family Medicine.
- New York State Medical License (or eligibility for licensure).
- Current DEA registration.
- Strong communication and interpersonal skills.
