In accordance with our 5-Star Quality Measure, our goal is to continuously maximize our efforts to ensure successful completion of the measure. The Quality Coordinator will be a vital component in successfully meeting our 5 Star measure. The candidate will need to have a high level of analytical and communication skills, attention to detail, ability to multi-task and meet deadlines
Duties and Responsibilities:
- Reconcile patient visits and claim completion
- Request medical records
- Update 5 Star patient gap list
- Immediately inform supervisor of gap list discrepancies
- Re-direct patients in network

- Reports
- Unable to reach patients
- Visit and claim verification
- Patient outreach
- Gathering of specific gap list details

Education & Experience:
- Associates degree preferred
- Must have intermediate to advanced MS Excel skills and comfortable with Excel formulas and tools
- Experience in a health care environment a plus

- Ability to communicate effectively verbally and through written communication with a variety of professionals
