About the Role
We are seeking an experienced Registered Nurse Hospice Case Manager for a 13-week contract assignment supporting hospice patients in the Vermont and New Hampshire service area. This role focuses on comfort care, pain management, patient advocacy, and care coordination for patients with terminal or chronic illness.
Responsibilities
- Provide hospice nursing care to patients in home and community-based settings
- Develop and update individualized patient care plans
- Coordinate care with physicians, families, caregivers, and interdisciplinary teams
- Assess clinical, functional, and emotional patient needs
- Manage pain control, comfort care, and end-of-life symptom support
- Educate patients and caregivers on care techniques and symptom management
- Document patient evaluations, progress, and care updates accurately in Epic
Requirements
- Active Registered Nurse license in Vermont and New Hampshire, or compact license required
- Graduate of an accredited nursing program
- Current BLS certification required
- Minimum 2 years of nursing experience required
- Hospice, home health, medical/surgical, or oncology nursing experience preferred
- Strong communication, compassion, and case management skills required
- Must be comfortable working independently while collaborating with an interdisciplinary care team
