Responsibilities:
The Ambulatory Pharmacy Clinical Coordinator is responsible for overseeing the coordination of clinical pharmacy services and providing patient care in the ambulatory pharmacy space, including referral-based HOPD pharmacist-managed clinics, specialty pharmacy services, and population health programs. This role involves collaborating with prescribers, providing leadership for clinical pharmacy initiatives, and supporting direct patient care efforts through medication therapy management. The Clinical Coordinator ensures effective communication across the pharmacy teams and prescribers, drives clinical service improvements, and oversees documentation of clinical interventions within electronic medical records (EMR).
Qualifications:

Required qualifications:

Current state Registered Pharmacist License.

B.S. in Pharmacy or PharmD degree required.

PGY-1 Pharmacy Practice Residency required (significant experience may be considered in lieu of residency).

5 years of previous ambulatory pharmacy experience required.

Board certification (BCPS, BCACP, or equivalent) within 18 months of hire required.

Strong verbal and written communication skills required.

Strong customer service skills required.
Preferred qualification:
-
PGY-2 residency in ambulatory care or a related specialty preferred.

Expertise in 340B program operations and compliance preferred.
-
Proficiency in EPIC or similar EMR systems preferred.

Advanced clinical knowledge in specialty areas such as diabetes, cardiology, pulmonology, rheumatology, or oncology preferred.

Experience as a pharmacy residency preceptor in ambulatory pharmacy services preferred.

A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $120,848.00 to $175,052.80 on an annualized basis. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
