Position Summary:
The NY Connects Program provides Information and Assistance and Person-Centered Options Counseling to individuals seeking access to Long-Term Services and Supports (LTSS). The Outreach Specialist is responsible for promoting NY Connects services throughout Brooklyn, developing and maintaining relationships with community partners, and strengthening referral networks. This position requires experience in community outreach, delivering presentations, and building effective partnerships among agencies and community organizations.
Essential Duties and Responsibilities:
- Identify, engage, and meet with community partner organizations that can provide referrals to and receive referrals from NY Connects, including disability service organizations, healthcare providers, government agencies, local Departments of Social Services, citywide coalitions, hospital social workers, home and community-based service providers, and other relevant organizations.
- Deliver presentations about the NY Connects Program to community stakeholders and represent NY Connects at community events, meetings, and other outreach opportunities.
- Collaborate with the Education and Outreach Coordinator to develop, assess, and monitor outreach strategies and activities.
- Build and maintain strong relationships with referral partners through ongoing communication, follow-up, and engagement.
- Complete timely, accurate, and standardized documentation and reports regarding outreach activities conducted on behalf of NY Connects and CIDNY.
- Share information and insights gained through community outreach to strengthen staff knowledge of programs, services, and resources available to consumers.
- Stay informed about community events, initiatives, and engagement opportunities relevant to Brooklyn and other assigned areas.
- Serve as an outreach liaison to borough managers to support and enhance outreach initiatives and identify opportunities for collaboration.
- Contribute to the NY Connects Resource Directory by identifying and sharing new or updated information about community resources and services.
- Maintain consumer confidentiality and ensure compliance with HIPAA, CIDNY policies, and all applicable privacy and data protection requirements.
- Attend agency trainings, staff meetings, unit meetings, and other required agency activities.
- Work occasional evenings and weekends as required for outreach activities and community events.
- Perform other program-related duties as assigned.
Education and Qualifications:
- Bachelor’s degree and at least two (2) years of experience conducting community outreach; or
- Associate’s degree and at least five (5) years of experience conducting community outreach.
Required Skills and Experience:
- Excellent written, verbal, presentation, and interpersonal communication skills.
- Strong computer skills, including proficiency with Microsoft Office applications and internet-based research.
- Ability to learn and effectively use new software applications and database systems.
- Ability to work independently while maintaining effective communication and collaboration with colleagues and community partners.
- Demonstrated ability to work respectfully and effectively with individuals from diverse backgrounds.
- Understanding of or experience with disability-related issues, programs, and services.
- Knowledge of Long-Term Services and Supports (LTSS) and public benefits.
- Experience working with community-based organizations, government agencies, local Departments of Social Services, and/or citywide coalitions.
- Strong organizational and time-management skills, with the ability to manage multiple outreach activities and follow-up responsibilities.
Preferred Qualifications:
- Bilingual proficiency.
Work Location and Travel Requirements:
This is a hybrid position requiring three (3) days per week in the office. The position is based in CIDNY’s Brooklyn office and primarily serves communities throughout Brooklyn, with other assigned areas as needed.
Regular travel throughout Brooklyn is required for meetings, presentations, community events, and other outreach activities. Occasional travel to other New York City boroughs may also be required.
