The LPN/RN Care Coordinator ? Phone Triage performs clinical specialty care coordination to our patients, providing health advice over the phone through compassion, empathy, and a strong desire to make a difference. The Triage Nurse will assess and triage patient symptoms to assist in obtaining the appropriate level of care and/or self-care advice and documents calls as required. An integral part of our care team, the Nurse Triage team is a trusted liaison for our patients, our providers, and our facility partners ensuring each patient feels comfortable and informed about the next steps in their care. Our team of 14 Providers deliver high quality care in a fast paced environment, so our people need to be agile and adaptable to the growing needs of our practice to deliver the same.
