Job Title: Children, Young Adults, and Families Specialist - Savannah
Pay Grade: I
Workplace: Region 5 Field Office - Hybrid: 3-5 days per week in office and/or traveling
Travel: 60-70%
Job Summary:
Serves as the Children, Young Adults and Families (CYF) resource for the Regional Office and participates as such on internal and external platforms and initiatives

Provides assistance and education on CYF focused services within the DBHDD Behavioral Health System and continuum of care.

Helps address challenges for agencies and individuals needing to access the system, with priority focus of DBHDD core customers.

Facilitates and/or participates in Local Interagency Planning Teams (LIPTs).
-
Trains new LIPT Chairpersons on roles and responsibilities.
-
Assist with coordinating and/or facilitation of regional CYF collaborative meetings.
-
Participates in Regional Community Collaboratives and lend expertise to youth focused committees.

Provides technical assistance to, participates in program reviews of, and promotes quality services for all C&A Providers with additional focus on:
- C&A Crisis Stabilization Units (CSUs)
- Psychiatric Rehabilitation Treatment Facilities (PRTFs)
- APEX
- IC3 – Intensive Customized Care Coordination
- Mental Health Resiliency Clubhouses
- Intensive Family Intervention (IFI) Services

Attend Carelon’s ASO Behavioral Health Quality Reviews and other audits, site visits, inspections, etc., as assigned.

Complaint resolution - follows up on grievances from constituent services as directed and ensure they are properly documented in Zendesk in a timely manner.

Participates in court hearings as directed by RSA/ DBHDD Legal Department.
-
Assist with transitioning youth from providers that are closing/terminated.
-
Assists with discharge planning reviews as needed and ensures youth transitioning to the community are connected to proper resources.
-
Attends family team meetings and transition planning meetings on cases that need regional input to offer assistance on available resources.
-
Collaborates with Regional Crisis Services Specialist to provide oversight of CYF Crisis System including daily monitoring of the Crisis Services Platform (CSP) and assists with linkage of children to crisis service providers as needed.

Collaborates with RFO DD staff on identifying services for children who are dually diagnosed and in need of Behavioral Health services.

Participate in regional, statewide, and otherwise located meetings, trainings, and conferences, which may require extensive travel, as approved and assigned by RSA or Regional Director.

Collaborate with other team members and assist with other regional office BH operational needs as requested and assigned.

Job Responsibilities:

Adheres to DBHDD Policies and Procedures: Has been advised of and adheres to all established policies, procedures, rules, and regulations as a condition of employment. Understand the responsibility of being aware of the content of DBHDD Human Resources policies, including, but not limited to, the Standards of Conduct and Ethics in Government, Smoking, Unlawful Discrimination, Sexual Harassment, Use of State Property, and the Drug Free Workplace policies.

Serves as RFO team leader for CYF services. Provides direction to community, BH team, and central office related to area of specialty.

Assesses and meets the needs of the community as it relates to CYF services and other areas as designated by supervisor. Provides feedback to team about unmet needs of individuals in those services.

Performs job duties with minimal supervision and independently seeks supervision when needed.
-
Monitors, assesses, and evaluates programs to determine compliance with state and/or federal guidelines and progress in meeting established goals. Informs BH team about effectiveness of continuum of care as it relates to specialty area as well as feedback about service gaps.

Researches, recommends, and utilizes best practices as related to specialty service areas. Compiles reports of findings and submits in appropriate format.

Identifies the need for training/technical support. Plans, coordinates, and implements educational, training, or technical support activities related to specialty areas. Provides policy and/or programmatic guidance to providers and DBHDD.

Establishes and maintains relationships with internal and external stakeholders to meet individuals and community needs. Organizes stakeholders to identify proactive and effective solutions to improve service provision and avoid duplication of services.

Participates in the determination of organizational policies regarding such issues as participant eligibility, program requirements and program benefits. Shares recommendations and guidance about policies with BH RFO team and DBHDD Central Office.
