Position Summary
The Care Coordination RN is responsible for all nursing-related enrollment work and for leading clinical discharge planning and bedside representation for PACE participants across home, hospital, clinic, and post-acute settings, including conducting in-home assessments as part of participant enrollment. This role serves as a key liaison between hospital teams, PACE IDT members, and community providers to ensure safe, timely, and coordinated transitions of care across all care environments. The Care Coordination RN promotes continuity of care by proactively identifying clinical risks, addressing gaps in care plans, and ensuring participants’ needs are effectively communicated and supported throughout the transition process.
Responsibilities:
- Complete all initial nursing assessments for new PACE enrollments
- Complete Semi-Annual or Change of Condition Assessments as indicated by IDT
- Serve as primary RN contact in hospitals and SNFs and other post-acute care facilities
- Coordinate clinical discharge planning and medication reconciliation in collaboration with the Social Work Department
- Communicate clinical changes to RNCMs
- Provide structured handoffs at transition completion
Requirements
- Bachelor's degree in Nursing, or obtained within five (5) years from the initial date of hire; RN with 15+ years of service may be considered in lieu of education agreement
- Advanced degree in Nursing (MSN, PhD, DNP, ARNP) preferred
- Current Registered Nurse license in Washington State or Multistate License endorsement (MLS)
- Current BLS Certification
- Three (3) years of recent clinical experience in specialty area

Pay and Benefit Expectations
We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $109,262.40 - $194,251.20 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant’s years of experience align.
