Family Medicine APP
South Jordan Clinic - Intermountain Health
- Competitive Compensation.
- Incentives: Up to $7,500 relocation bonus (if applicable).

- Clinic Hours: Tuesday 8 am- 5:30 pm - West Jordan, Wednesday, Thursday, Friday at South Jordan
- Current state license as a Physician Assistant or Advance Practice Registered Nurse (APRN).
- Certification as a Physician Assistant or Nurse Practitioner (Credentialing requirement).
- Eligible for State Controlled Substance License II through V.
- Eligible for Federal DEA Certificate II through V.
- BLS Certification for Healthcare Providers.
- Other certification as required (e.g. ACLS, PALS, TNCC, NRP).
Preferred Qualifications:
- Two (2) or more years of experience preferred.
- Master’s degree. Education must be obtained from an accredited institution. Degree will be verified.
