JOB SUMMARY:
We are seeking a passionate and skilled Medical Provider to join our growing team. This role is ideal for a provider who thrives in a supportive, forward-thinking environment and is committed to whole-person care. RESPONSIBILITIES AND DUTIES:
- Provide direct patient care through office visits, telehealth, and occasional same-day appointments
- Perform comprehensive assessments, order and interpret labs/imaging, and diagnose acute and chronic conditions
- Develop personalized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescriptions
- Educate and empower patients on preventive health and functional medicine principles
- Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff
- Document visits in the EHR (NextGen) in a timely and compliant manner
- Participate in team meetings, quality improvement initiatives, and training sessions
- Support clinic operations by contributing to a positive, team-oriented environment
REQUIRED QUALIFICATIONS
- Active Oregon license as a Certified Physician Assistant (or eligibility to obtain)
- Board certification in Family Medicine or Internal Medicine
- Strong interest in preventive, functional, and lifestyle medicine
- Excellent communication and patient relationship skills
- Comfortable working in a fast-paced, collaborative environment
- Proficient in using electronic health records
