Base Location: Lawrenceville, GA/Gwinnett Co.
Division/Department/Program: Adult Crisis Stabilization Unit (CSU)
Additionally, we offer a Temporary Observation Unit (TOU) at the Adult Crisis Stabilization Unit. In our TOU area, we provided evaluation services within a 23-hour period to determine whether an admission to the Crisis Stabilization Unit or inpatient psychiatric facility is needed or if the client would be able to be stabilized with more intensive outpatient (rather than inpatient/hospital) service setting. During time on TOU a client may or may not be on an involuntary status and will be assessed by a nurse, physician, and clinician. A paraprofessional and CPS may also provide services during this time to assess the clients’ overall needs and level of care. In addition to the initial assessment, clinicians may also provide crisis intervention services, individual and family counseling services as well as case management in order to assist in determining client plan at end of their 23 hours stay.
The TOU Clinician will work with adult clientele placed in the Temporary Observation Unit. The Clinician will provide comprehensive assessment and treatment planning. Clinician will provide individual counseling to identify triggers for crisis behaviors, process emotions, and work toward coping skills identification. Family counseling services will also be offered to engage family into treatment, proactively address conflicts/barriers for care, and assist with safety planning in the home. Case management services will also be provided to link with meaningful resources that support the client having safe placement and aftercare services that address behavioral health and other needs. The Clinician will assist/partner with the multi-disciplinary team, in developing plans of care all while cooperatively working to ensure smooth transitions to other levels of care. This position is responsible for providing billable services and will be responsible for meeting set minimum billing goals. This position is also eligible for incentive-based pay (see below).

The Care Coordinator will assist/partner with the multi-disciplinary team in developing transition/discharge plan of care all while cooperatively working with community providers to ensure smooth transitions to next level of care.
Duties & Responsibilities:
- Assist with addressing failed claims/activities and daily review of Carelogic alerts.
- Schedule aftercare and follow up appointments (behavioral health intake, counseling, psychiatric, primary care, etc.) for all clients on caseload and any others assigned.
- Identify and confirm placement for clients that are homeless or in need of residential treatment or other treatment programs. This may include scheduling, assistance with application paperwork, coordination with treatment facilities, and facilitating interviews between client and facility.
- Establishes comprehensive resource guide and materials for ACSU and TOU clientele that is stored electronically for all clinical team to access.
- Establishes and maintains resource connections with external partners and community organizations, manages referrals procedures, initiates and tracks linkage to care including discharges.
- Develops and maintains close working relationships with community service providers.
- Attend treatment team daily to discuss client progress and recommendations based on services provided. If you are not available to attend, provide handoff report to supervisor.
- Coordinate with clinician assigned to clients on caseload to ensure best care.
- Complete all documentation within 24 hours of service and adhere to all documentation guidelines and service definitions for interventions for services to appropriately be billed.
- Complete and track data for aftercare calls of all clients following procedure outlined in the CSU Suicide SOP addendum. Complete all related documentation for calls.
- Complete daily discharge communication call to the outpatient centers for all discharging clients (per suicide SOP).
- May be required to transport clients in View Point Health Vehicles.
- Comply with all Viewpoint Health company policies specific to the employee manual and additional site-specific guidelines.
- Facilitates referral/transfer process to inpatient psychiatric hospitals for insured clients admitted to TOU that require continued crisis level care. This may include calling to assess bed availability, gathering and faxing information required for referral, scheduling transportation, ensuring nurse to nurse completion, etc.
- Support in completion of authorization requests for TOU.
- Other related duties as assigned.
Minimum Qualifications:
High School diploma/GED and three (3) years of job-related experience in a social services-related position; or One (1) year of experience at the lower-level Social Services Tech 2 (SST011) or equivalent position.
Preferred Qualifications:
- Bachelors degree in social services related field from an accredited college or university
- One (1) years’ experience in Social Services related field.
- Experience with CareLogic or other ECR/EMR.
- At least 2 years’ experience working in behavioral health care.
- Previous work experience in a crisis stabilization, inpatient setting or ACT team experience.
- Bilingual - ability to read, write, and speak in Spanish and English
Requirements/Competencies:
- Must be able to lift 20 pounds.
- Requires long periods of sitting/standing.
- Must have valid Georgia driver’s license and Motor Vehicle Record in accordance with company policy.
- Candidates for selection must pass a criminal background check (including fingerprinting).
