Job Description
We seek a self-driven and organized Patient Care Coordinator to join our team. This individual will play a key role in managing patient outreach, telehealth schedule coordination, and delivering exceptional customer service. The position requires a hybrid work model, requiring in-office alongside set work from home days.

Key Responsibilities
- Coordinate patient care and ensure timely scheduling of appointments.
- Create and maintain our exclusive network of Clinical Network Providers' telehealth schedules.
- Serve as the primary point of contact for patients.
- Execute outbound phone calls to eligible and active program patients.
- Manage multiple workflows and collaborate across different departments.
- Collaborate with account management and contracted strategic partners to maximize patient onboarding success
- Maintain organized records and documentation of patient care.
- Assist in streamlining operational processes for optimal efficiency.
- Provide excellent customer service to patients, healthcare providers, and staff.
- Support clinical team members, leveraging clinical knowledge to enhance patient care coordination.
- Attend meetings and trainings as requested.
Qualifications
- Strong organizational and multitasking skills.
- Familiarity with HIPAA compliance rules.
- Excellent communication and customer service abilities.
- Ability to handle diverse tasks in a fast-paced environment.
- Advanced in Excel and online platform management.
