Home Health Professionals Network (HHPN) is seeking motivated Registered Nurse Case Managers to provide skilled, physician-directed care throughout Hartford and surrounding communities.
The RN Case Manager will deliver high-quality nursing care, coordinate services, educate patients and families, monitor outcomes, and work closely with the HHPN clinical team to ensure every patient receives the right care at the right time.
Key Responsibilities:
- Conduct skilled nursing visits and comprehensive assessments
- Develop, implement, and monitor patient-centered plans of care
- Administer medications and treatments as ordered
- Monitor patient conditions and report changes
- Teach patients and families about medications, diagnoses, and health management
- Coordinate care with providers and interdisciplinary team members
- Complete timely and accurate clinical documentation
Qualifications:
- Active Connecticut Registered Nurse license
- Home health or homecare experience preferred
- Current BLS certification
- Strong clinical, communication, and organizational skills
- Reliable transportation
- Ability to work independently and manage time effectively
- Compassionate and professional demeanor
