We are looking to fill two roles currently. They are:
1. Telehealth Provider (1099 Independent Contractor)
We are seeking a licensed provider to deliver patient-centered care through telehealth. In this role, you will conduct clinical assessments, develop individualized treatment plans, provide services within your scope of practice(i.e. medication management), maintain timely and accurate documentation, and coordinate care as needed.
The ideal candidate is dependable, comfortable with virtual care technology, and able to work independently while communicating clearly with patients and our administrative team. Applicants must hold an active license appropriate to their discipline in the state where patients receive care.
This is a 1099 independent contractor position. We are looking for a PA-C or PMHNP-BC.
2. Assisted Living Facility Provider 1099 Independent Contractor
We are seeking a licensed provider to deliver on-site care to residents in an assisted living facility. In this role, you will conduct assessments, develop individualized treatment plans, provide ongoing services within your scope of practice, complete timely clinical documentation, and collaborate with facility staff, families, and other treating professionals as needed.
The ideal candidate is compassionate, organized, and comfortable working with older adults, with the ability to support residents through a range of behavioral health needs, primarily medication management. Experience in geriatric behavioral health or long-term care is preferred.
This is a 1099 independent contractor opportunity. Seeking a PA-C or PMHNP-BC
