Salary Range: $35.46 - $50.05 per hour

ROLE AND RESPONSIBILITIES
How you'll make an impact in this role:
- Delivered skilled home-based nursing care to PACE participants, including assessments, acute triage, wound care, blood draws, medication management, and chronic disease monitoring.
- Conducted in-home visits and coordinated care across healthcare providers, assisted living, and long-term care settings to support aging in place and reduce hospitalizations.
- Collaborated with the Interdisciplinary Team (IDT) to develop and implement participant-centered care plans that addressed medical, functional, and psychosocial needs.
- Educated participants and caregivers on disease management, medication adherence, safety, and end-of-life care while providing after-hours and on-call nursing support.
MINIMUM QUALIFICATIONS

Licensure / Certification / Registration:
- Required Credential(s):
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.

Registered Nurse obtained prior to hire date or job transfer date.

Education:
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
