Job Description:
The incumbent will work as part of an interdisciplinary team, serving individuals with severe and persistent mental illness, including those with co-occurring disorders. Incumbent will act as a Care Manager in performing assessments, helping develop individualised recovery plans, determining the appropriate level of care, placing individuals in those levels of care and then ensuring that care is delivered according to standards. Other duties will include visiting beneficiaries in the community, charting, and acting as a liaison with a large network of community providers.
