About the Role
We are seeking a compassionate, organized, and detail-oriented Adult Foster Care (AFC) Case Manager to join our team serving the South Shore region of Massachusetts. In this field-based role, you will work closely with participants, family caregivers, and multidisciplinary healthcare teams to ensure high-quality, person-centered care in a home setting.
Key Responsibilities
- Conduct regularly scheduled in-home visits to evaluate participant health, safety, and overall well-being.
- Collaborate with Registered Nurses (RNs) and primary care physicians to build, implement, and review multi-disciplinary care plans.
- Support caregivers by providing resources, training, advocacy, and guidance.
- Maintain strict compliance with MassHealth Adult Foster Care regulations, completing timely and thorough clinical documentation and progress notes.
- Connect participants to local community resources, medical supply programs, and specialized transportation services.
Service Area
This position requires travel throughout Norfolk and Plymouth Counties. Service cities and towns include:
- Coastal & Northern South Shore: Quincy, Braintree, Weymouth, Hingham, Hull, Cohasset, Scituate, Norwell, Marshfield, Duxbury
- Inland & Metro-South: Milton, Randolph, Holbrook, Brockton, Abington, Whitman, Rockland, Hanover, Hanson, Pembroke
- Southern Plymouth County: Kingston, Plympton, Plymouth, Carver, Halifax, Bridgewater, East Bridgewater, West Bridgewater
Qualifications
- Education: Bachelor’s degree.
- Experience: Minimum of 1 year of clinical experience working with elderly individuals or adults with physical or developmental disabilities.
- Licensure/Certifications: Valid Driver’s License, reliable personal vehicle, and active auto insurance required.
- Skills: Strong organizational habits, excellent written/verbal communication, and proficiency with Electronic Health Records (EHR) software.
- Knowledge of MassHealth AFC regulations is a plus.
Pay: $26.34 - $31.72 per hour
