Job Summary
At North Central Nursing Clinics (NCNC), a federally qualified health center (FQHC), you'll have the opportunity to make a meaningful difference by expanding access to high-quality healthcare and improving the well-being of the communities we serve.
Reporting directly to the Medical Director, the Clinical Quality & Performance Lead / Nurse Practitioner provides primary healthcare services to NCNC patients and serves as a clinical leader for organizational quality improvement, population health, and value-based care initiatives.
This hybrid position dedicates approximately 50% of its responsibilities to direct patient care and 50% to clinical quality and performance improvement. In the clinical quality role, the position collaborates with the Medical Director and the Director of Quality to translate organizational quality priorities, clinical performance data, regulatory requirements, and value-based care objectives into effective clinical practice.

North Central Nursing Clinics (NCNC) is a federally qualified health center (FQHC) that provides access to high-quality primary healthcare services for the communities it serves.
This position is primarily located in Delphi, IN, it may require occasional travel to the Burlington, Monon, and/or Wolcott clinics as needed for coverage.
Responsibilities Include:
Serve as a provider-level resource and liaison between NCNC's clinical providers and the Quality/Compliance team.
Helping identify barriers to clinical performance, develop and implement evidence-based interventions, promote consistent clinical workflows, and engage providers in continuous improvement.
Supports improvements in clinical quality measures across the Health Resources and Services Administration (HRSA) Health Center Program, Uniform Data System (UDS), payer and value-based care programs, and other organizational quality initiatives.

Required:
Education and Experience:
Master of Science degree in Nursing with specialization as a Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner.
Advanced degree as a Nurse Practitioner.
Verified certification to practice in the state of Indiana and a completed background check.
One year of experience as a Registered Nurse.
Indiana State RN and APN licensure.
Certification as a Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner by national certifying body.
What is Helpful:
Additional years of experience.
Experience with value-based care (VBC) and HEDIS metrics, Data capture for electronic clinical quality measures (eCQMs).
Experience with MSSP ACO population health measures.
