Job Overview

We are seeking a dedicated and compassionate Registered Nurse (RN) to join our hospice and home health care business. In this vital role, you will provide exceptional patient-centered care, coordinate clinical services and collaborate with a multidisciplinary team to ensure the highest quality of care for our patients. Your expertise will help facilitate smooth transitions between hospital, outpatient, and home settings, ensuring patients receive compassionate support throughout their healthcare journey. Responsibilities

• Conduct comprehensive patient assessments to determine care needs and develop individualized care plans.
• Manage patient care across various settings including hospice, home health, outpatient clinics, and inpatient facilities while working from the office and field.
• Collaborate closely with healthcare professionals such as physicians, social workers, therapists, and case managers to coordinate treatment plans and ensure seamless patient care.
• Perform clinical case management tasks in our office including discharge planning, medical records documentation, and utilization management.
• Utilize electronic health records (EHR) systems to accurately document patient information, progress notes, and treatment updates while maintaining HIPAA compliance.
• Provide education and support to patients and families regarding disease management, medication administration, and end-of-life care options in hospice settings.

Skills

• Skilled in managing diverse staff and patient populations in the office and in the field.
• Critical care experience with a strong understanding of physiology and complex patient conditions.
• Experience working in Level I or Level II trauma centers, emergency rooms, ICU units, or hospital medicine environments.Skilled in managing diverse staff and patient populations in the office and in the field.
• Proficiency in medical terminology, EMR/EHR systems, discharge planning, and medical records management.
• Excellent patient service skills with the ability to communicate effectively with clients and collaborate with healthcare teams.
• Familiarity with managed care practices, utilization management processes, diagnostic evaluation techniques, and HIPAA regulations for privacy protection.

Pay: $35.00 - $90.00 per hour
