INTRODUCTION
The Scheduling/ Client Care Coordinator reports directly to the General Manager and is responsible for guiding and coordinating all scheduling and client relationship-related activities of the SYNERGY HomeCare business. This requires organizing schedules to accommodate both clients and caregivers, collaborating with the franchise partner and administrator to enhance employee satisfaction, and client check-ins/issue resolution. It is the duty of the Client Care Coordinator/ Scheduler to maintain positive relationships with caregivers and clients. At times, this position requires you to fill in for night-time and/or weekend schedulers and caregivers as needed.
BASIC QUALIFICATIONS
- Knowledge of scheduling and on-call procedures (preferred)
- Possess at least one year of experience in healthcare or home care, working with the elderly, disabled or individuals requiring supportive services (preferred)
- Proficiency in computer skills (Google Suite, Excel, PowerPoint, etc)
- Knowledge of scheduling software (AxisCare) or CRM preferred
- Must possess leadership skills, organizational skills, customer satisfaction skills, and human relations abilities
- Well-honed time management skills
- Exemplary communication skills, both verbal and written
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Regular communication with caregivers
- Regular communication with clients and their families
- Onboarding of clients to include a complete and thorough transfer of data to schedule
- Retention planning for client’s service
- Updating files and maintaining current client assessment information
- Assist with onboarding new employees, as needed
- Conducting on-site training with the client to ensure caregivers are knowledgeable on how to care for the client
- Record all home assessment information correctly and completely in AxisCare
- May be asked to participate in an on-call rotation
- Submit weekly reports for the staff meeting
- Assist with phones and other in-service training
- Conduct Home Assessments and follow up on client issues
