SUMMARY
To perform duties and render such services required to maintain the health and physical welfare and potential rehabilitation of NHVH residents. To develop and implement individualized therapeutic interventions and treatment plans based on nursing process and standards of practice.

Each additional year of approved formal education may be substituted for one year of required work experience and/or each additional year of approved work experience may be substituted for one year of required formal education.
MINIMUM QUALIFICATIONS:
Education/Experience: Bachelor's degree and 4 years of experience OR equivalent combination of 8 years of education and experience after completion of high school. Must include at least 30 credits of graduate-level study in nursing with a specialty in geriatric, family practice, or psychiatric nursing.
License/Certification:
NH Advanced Practice Registered Nurse (APRN) license
Drug Enforcement Administration (DEA) certification

Responsibilities:
DETAILED TASKS (AGENCY/POSITION-SPECIFIC):
- Performs comprehensive physical examinations, psychological/mental health assessments, and obtains additional information from residents/families, consultants, and other providers in order to arrive at a diagnosis and plan of care.
- Maintains the necessary charts, orders, and other adequate records.
- Calls Activated DPOAHC (designated power of attorney health care) as appropriate.
- Enters orders and progress notes into electronic health record.
- Signs all telephone orders as appropriate.
- Identifies, isolates, and treats residents with reportable diseases and notifies proper public health care agencies.
- Orders, analyzes and interprets laboratory and diagnostic tests to include X-rays, EKG's, MRI's, CT’s, and EMG’s, and prescribes, administers and modifies pharmacologic and psychopharmacologic medications and treatments according to established protocols and within the scope of practice.
- Compiles, prepares, and presents medical case histories and clinical summaries and acts as a contributing member of the interdisciplinary team engaged in providing treatment for acute and chronic behavioral, emotional and/or physical disturbances.
- Embraces leadership core competencies such as effectively liaising between various parties to improve healthcare and translate new knowledge into practice through leadership.
- Applies clinical expertise and facilitates learning core nursing competencies in the Long-Term Care Environment.
- Develops and provides continuing education for nursing staff to improve patient outcomes.
- Organizes, supervises, and reviews the work of subordinate professionals, para-professional personnel engaged in patient/resident care and treatment.
