We’re seeking a dynamic Director of Post-Acute Partnerships to expand referral partnerships, strengthen hospital relationships, and drive census growth across our skilled nursing centers. What You’ll Do
As a trusted liaison between hospitals, healthcare providers, and our skilled nursing centers, you will:
- Develop and strengthen relationships with hospitals, physicians, case managers, discharge planners, accountable care organizations, and community partners.
- Identify new referral opportunities while maintaining and growing existing partnerships.
- Coordinate patient referrals from initial clinical review through admission, ensuring seamless transitions of care.
- Collaborate with admissions, nursing, and executive leadership to deliver outstanding patient and customer experiences.
- Promote the clinical capabilities and specialty programs that set our centers apart.
- Leverage CRM technology and market data to identify growth opportunities and measure performance.
- Create and execute strategic business development plans that increase admissions and market share.
- Analyze referral trends, conversion metrics, and market intelligence to continuously improve results.

Preferred qualifications include:
- RN or LPN license preferred (where applicable), but not required.
- Valid driver’s license with the ability to travel locally.
- Five or more years of clinical, healthcare sales, business development, case management, or post-acute care experience.
- Experience working with hospitals, managed care organizations, or skilled nursing facilities is highly desirable.
- Excellent communication, presentation, and relationship-building skills.
