ROLE AND RESPONSIBILITIES
A Non-Medical Care Manager contributes to AHF by providing coordination, guidance, and assistance in improving access to and retention of needed medical support to mitigate and eliminate barriers to HIV care services. With a caring and knowledgeable focus, you will work with our providers and healthcare center team so that every Ryan White client receives the support they require and access to available resources.
- Completing Initial Assessments and developing Individualized Care Plans related to psychosocial needs and goals.
- You will draw on your experience and skills in understanding basic needs such as housing, food, and clothing, develop relationships with programs and agencies serving those needs, and facilitate referrals, support, and assistance for your clients who lack those services.
- In this way, you will support the client's ability to better understand their health status so that they can work to achieve their healthcare goals.
- You will also conduct ongoing retention calls to ensure no gap in retention efforts and to uphold radical cooperation across divisions.
A Non-Medical Care Manager enhances the reputation of AHF by utilizing your case management experience, you will meet our clients where they are, identify strengths and opportunities for growth, and assist them with realistic goal setting.
- With your skills of collaboration and engagement, you will bring the essence of the Ryan White Care Act - the safety net for uninsured HIV/AIDS patients - to our most vulnerable population.
- Your needs assessments, care plans, referrals, and support will directly contribute to AHF's success in meeting the deliverables of our Ryan White contract.
- Beyond the scope of the contract, your work contributes to measures established by the HIV/AIDS Bureau (HAB) Measures and the Department of Health and Human Services.
- You will verify the client's eligibility for the ACA (Affordable Care Act) program and enroll those who qualify to ensure broader access to healthcare services.
You will be a passionate advocate for our top initiatives. To be successful as a Non-Medical Care Manager it is crucial to ensure that the AHF core Values and Mission stay top of mind with all that your do. At AHF we are nimble and able to adapt in a dynamic environment to assist in providing the best experience for our clients and workplace for our employees.
AHF has a collaborative organizational structure where staff are accountable to multiple leaders. The Non-Medical Care Manager will work as a part of a close knit team to collaborate with the provider and patient and is able to facilitate the shift from leaving the exam room to implementing the optimum plan of care for these non-medical needs and putting it into action.
- Providing brief interventions focused on a long-term achievable goal, the patient and the Non-MCM work together each step of the way towards viral suppression, better health outcomes and quality of life.
BASIC QUALIFICATIONS
- High School Diploma required.
- Bachelor degree in health or human services-related field preferred.
- A minimum of 1 year Case Management experience is preferred.
- Knowledge of Ryan White Services is preferable.
