Job Summary

Alma Women’s Health Collective is seeking a compassionate, collaborative, and motivated Behavioral Health Care Manager (BHCM) to join our growing team. This role is an opportunity to work at the intersection of behavioral health and women’s health, supporting patients through a collaborative, whole-person model of care.

We are especially interested in candidates who have experience working with women and women's health, or who have a strong professional interest in developing expertise in this area. Experience with perinatal mental health, reproductive health, fertility, postpartum care, menopause, trauma, anxiety, depression, or other behavioral health concerns affecting women is a plus.

The BHCM will be a core member of our Collaborative Care Model (CoCM) team, working closely with patients’ medical providers, a psychiatric consultant, and, when appropriate, licensed behavioral health clinicians such as an LCPC, LCSW, LMFT, or similar provider.

The BHCM will be responsible for supporting and coordinating the mental and physical health care of an assigned patient caseload. This includes engaging patients, coordinating care, monitoring progress, supporting treatment plans, and ensuring strong communication among members of the care team.

This position is also an opportunity to help build and shape the behavioral health program at Alma. The successful candidate will be comfortable not only providing effective care management, but also developing new processes, communication systems, educational resources, workflows, and best practices alongside our growing behavioral health team.

BHCM Duties & Responsibilities

• Provide compassionate, patient-centered behavioral health care management for an assigned caseload, supporting both the mental and physical health needs of patients.
• Coordinate closely with patients’ medical providers and, when appropriate, therapists, psychiatrists, and other members of the behavioral health team to ensure integrated care.
• Screen and assess patients for common mental health and substance use concerns using validated screening tools.
• Develop strong, trusting relationships with patients and facilitate engagement, follow-up, and continuity of care.
• Provide education and psychoeducation related to mental health, behavioral health, and women's health using evidence-based resources and approved educational materials.
• Support patients experiencing behavioral health concerns across the lifespan, with particular attention to issues that may intersect with women's health, including reproductive and perinatal mental health, fertility, postpartum adjustment, menopause, hormonal changes, trauma, anxiety, depression, and other concerns as clinically appropriate.
• Systematically monitor treatment response, including changes in symptoms, functioning, treatment engagement, and potential medication side effects or complications.
• Conduct regular patient outreach through in-person visits, telephone, and/or other approved communication methods.
• Work collaboratively with patients to identify and address social determinants of health and other barriers to care, including food insecurity, transportation, housing, financial concerns, childcare, and access to community resources.
• Provide brief, evidence-based behavioral interventions and coaching as clinically appropriate, including behavioral activation, problem-solving treatment, motivational interviewing, and other approved approaches.
• Help patients develop skills for symptom awareness, coping, self-management, and long-term wellness.
• Provide or facilitate referrals to evidence-based behavioral health treatments and other specialized services when indicated.
• Coordinate referrals and follow-up with providers and organizations outside of Alma when patients require additional or more specialized care.
• Coordinate with larger support systems on a patient's behalf when appropriate, including schools, other medical providers, community organizations, and family members, while maintaining appropriate patient consent and confidentiality.
• Participate in regularly scheduled caseload consultation with the psychiatric consultant and communicate resulting treatment recommendations to the appropriate medical and behavioral health providers.
• Use the patient registry and other tracking systems to monitor engagement, treatment progress, and clinical outcomes.
• Proactively identify patients who may benefit from additional outreach or re-engagement in care.
• Maintain timely, thorough, and accurate documentation in the EHR, registry, and other required systems.
• Document patient progress, interventions, care coordination, and treatment recommendations in a manner that supports communication among the full care team.
• Work collaboratively with the CoCM team to identify patients who are not improving as expected and help facilitate changes to treatment plans or referrals for a higher level or more specialized level of care.
• Facilitate referrals to clinically indicated services, including specialty mental health care, substance use treatment, social services, housing resources, vocational resources, and other community-based supports.
• Develop individualized relapse-prevention and self-management plans with patients who have met their treatment goals and are preparing to transition out of active care management.
• Coordinate with Alma's billing team to ensure services are appropriately documented and billed.
• Contribute to the ongoing development of Alma's behavioral health program by helping create and refine workflows, communication systems, patient education materials, clinical processes, and best practices.
• Collaborate with the broader Alma team to identify opportunities to improve the patient experience and strengthen the integration of behavioral health into women's health care.
• Bring curiosity, initiative, and a willingness to help build new systems as Alma's behavioral health services continue to grow. Training & Licensure Requirements
• A bachelor’s degree in social work, psychology, counseling, health and human services, nursing, public health, or another behavioral or physical health-related field is preferred.

• Strong attention to detail and the ability to manage an assigned caseload while maintaining timely documentation and follow-up.
• The ability to work independently while also seeking guidance and collaborating closely with medical and behavioral health providers.
• Comfort working in an evolving environment and contributing ideas to improve workflows, communication, and patient care.
• A willingness to learn and participate in ongoing training and clinical consultation.
• Knowledge of HIPAA requirements and the ability to maintain patient confidentiality and privacy in accordance with all applicable regulations.

Work Environment

The Behavioral Health Care Manager will primarily work within Alma Women’s Health Collective's clinical environment and may provide care both in person and virtually, depending on patient needs and clinical responsibilities.

The BHCM will work collaboratively with medical providers, behavioral health clinicians, psychiatric consultants, and other members of the Alma team. The role may include time in designated office space, clinical areas, and other appropriate settings within our practice.

The position requires the ability to balance direct patient interaction, care coordination, documentation, team communication, and administrative responsibilities in a dynamic clinical environment.

• Equivalent education and/or relevant professional experience may be considered on a case-by-case basis.
• Licensure is not required for the Behavioral Health Care Manager position.
• Alma Women’s Health Collective will provide training, support, and ongoing professional development to help the successful candidate grow into the role.
• Prior experience in behavioral health or care management is preferred but not required. Strong candidates may have 2–3+ years of experience in behavioral health, social services, care coordination, patient navigation, case management, or a related field.
• Experience working with women, families, or within a women's health setting is a plus. We also welcome candidates who have a strong interest in women's health and are excited to develop expertise in this area.

Pay: $60,000.00 - $65,000.00 per year
