PAKT, LLC is currently seeking a Home & Community Based Case Manager. In this role, you will have the opportunity to support families through their toughest challenges, guiding them toward safety and stability. You will be responsible for providing home-based case management services and supervising court ordered parenting time for families involved with the Indiana Department of Child Services. Our focus is on fostering a supportive environment that promotes safety, healing and growth. This is a full time or part time contractual position. Responsibilities
- Provide a safe and structured environment where parents and children can connect, and healthy and nurturing relationships are supported. Services should be family centered and culturally competent.
- Strengthen family dynamics by intervening and role modeling appropriate interactions as needed.
- Assist parents with securing employment, adequate housing, transportation, utility assistance, childcare, and other community resources/services as needed in order to establish and maintain stability.
- Educate parents in child development, parenting, stress management, and the effects of child abuse and neglect.
- Build rapport and trust with the families we serve.
Education and Qualifications
- Must be 21 years of age.
- Bachelor’s or Master’s degree from an accredited university.
- Prior experience providing services to families needing assistance in the protection and care of their children is a plus.
- Must be able to respond professionally, thoroughly and objectively to the demands and pressures of the position.
- Excellent organizational, planning and reporting skills required.
- Excellent listening/verbal communication skills and confidentiality imperative.
- Ability to think critically during tough or stressful situations.
Required
- Follow DCS service standards while managing an independent caseload and ensuring all reports are submitted as required.
- Attend meetings and court hearings at the request of contracting agency and/or DCS.
- Safe transportation of clients and children, utilizing your own vehicle.
- Must possess a valid driver’s license with good driving record, minimum auto insurance coverage as per state policy and required personal/professional liability insurance.
- Must pass all required background checks, including fingerprinting, criminal and CPS.
- Travel to surrounding counties as needed in order to meet the referral requirements.
- Evening and weekend availability to meet the referral requirements.
