JOB SUMMARY AND QUALIFICATIONS
As a Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN) at HCA Healthcare, you’ll play a vital role in delivering compassionate, personalized care to patients in the comfort of their homes. In collaboration with the Registered Nurse and care team, you’ll help manage medical needs and promote their independence – caring for patients like family, one home visit at a time.
Your responsibilities will include:
- Providing nursing care in the home, including wound care, medication administration, and support during procedures
- Documenting assessments, interventions, and outcomes clearly and thoroughly to support safe, continuous care
- Monitoring patient status and alerting the care team of any changes to ensure timely, coordinated responses
- Educating patients and families to support medication adherence, strengthen understanding of health conditions, and promote greater independence at home
- Collaborating closely with RNs, providers, therapists, and care coordinators to deliver high-quality, personalized care in the home setting
What qualifications you will need:
- Graduate of an accredited LPN/LVN program
- Minimum of one year experience as an LPN/LVN in a clinical care setting
- Home care experience preferred
- Must be licensed as an LPN/LVN in good standing in the state(s) where they will be providing services
- Meets all additional requirements for hire including state specific requirements
- Current BCLS prior to delivering patient care
- Reliable transportation and proof of valid automobile liability insurance
- Must have valid driver’s license
