INTRODUCTION
Fairview is seeking a Registered Nurse (RN) Transplant Coordinator to join our deceased donor-Lung Transplant team, based at the Phillips-Wangensteen Building on the University of Minnesota East Bank Campus. This is a 0.8 FTE position (64 hours per pay period) and is benefit eligible.
The anticipated schedule is Monday through Friday, approximately 8:30 a.m. to 5:00 p.m., with one scheduled day off each week. In this role, you will provide specialized care coordination and support for post-transplant lung patients, collaborating closely with a multidisciplinary team to ensure optimal patient outcomes throughout the post-transplant journey.
The Transplant Coordinator is responsible for coordination and preparation of patients for organ transplantation and the long-term management of patients after transplant through collaboration with various members of the interdisciplinary team.
Responsibilities
- Coordinate patient care in both pre-transplant phases
- Collaborate with interdisciplinary team members within the organization and outside community partners
- Act as a liaison for transplant physicians to implement medical management of conditions
- Provide telephone triage and make patient care decisions based on patient symptom assessment and evaluation, laboratory results, and patient condition changes
- Apply appropriate medical interventions and transplant protocols based on patient assessment, laboratory results, and patient condition changes
- Assume on-call responsibilities after office hours by providing a resource for medical triage. Act as transplant coordinator when donor organ becomes available
- Provide ongoing education to patients and families
- Collaborate with other members of the clinical team to improve patient care and benchmarks through quality improvement projects
- Participate in the development and revision of protocols
- Maintain knowledge of current practices in transplantation, nursing, and healthcare
BASIC QUALIFICATIONS
- Bachelors Degree in Nursing or Associates Degree in Nursing
- If Bachelors Degree: 3-5 years acute care experience in ICU, CCU, Transplant, Acute Care Med-Surg
- If Associates Degree: 5+ years of acute care experience in ICU, CCU, Transplant, Acute Care Med-Surg
- MN Registered Nurse (RN) License
- Basic Life Support (American Heart Assoc or Red Cross)
QUALIFICATIONS
$100,068.80 - $144,539.20 Annual
