Position Summary:
The RN Case Manager: is responsible for coordinating and managing the nursing care of individuals with Intellectual and Developmental Disabilities (IDD) in the home setting. This role involves comprehensive case management, including assessing individuals’ needs, developing care plans, and ensuring coordination among interdisciplinary teams to ensure quality care for individuals with IDD.
The RN Case Manager will serve as the primary point of contact for individuals and their families, ensuring that all healthcare needs are met and that services are provided in a timely, efficient, and compassionate manner.
Key Responsibilities:
1. Care Coordination and Case Management:
- Assess the physical, emotional, and developmental needs of individuals with IDD in their home environment.
- Develop individualized care plans based on individual assessments, collaborating with Supports Coordinators, physicians, therapists, and other healthcare providers, as needed.
- Serve as the primary point of contact for individuals and their families, ensuring clear communication and coordination of services.
- Monitor individuals’ progress and make adjustments to care plans as needed, based on individual responses and evolving needs.
- Coordinate schedules for new and existing individuals to ensure staffing meets the appropriate level of care and the desired needs of the individual.
2. Referrals, Staffing Coordination and Onboarding:
- Referral Generation: Drive business by generating referrals from community partners for individuals with IDD with needed services and supports.
- Assess Staffing Needs: Work with RN Nursing manager and Nursing Business Operations manager to assess the staffing requirements for current and upcoming referral needs, ensuring the right balance of skills and expertise to meet service delivery needs for individuals with IDD.
- Referral Process Management: Oversee the referral process, ensuring that staffing referrals are timely, accurate, and aligned with individual needs and standards.
3. Collaboration and Communication:
- Communicate with the individual’s interdisciplinary care team, including Supports Coordinators, physicians, therapists, social workers, and caregivers, to ensure comprehensive, coordinated care.
- Educate individuals and families about the individual’s condition, care requirements, and strategies for managing health at home.
- Develop and implement strategies to address any concerns or barriers to effective care, ensuring individual and family satisfaction.
4. Compliance and Documentation:
- Monitoring and Oversight of Field based RNs and LPNs: Ensure that RNs and LPNs are submitting daily nursing notes that align with individual needs and outcomes.
- Staff Training: Provide training and guidance to field nurses, family members, and other team members on best practices in care delivery, person-centered planning, and internal policies.
- Maintain Documentation: Accurately document and maintain all required records, including ISP, assessments, nursing notes, and incident reports.
- Incident Reporting: Ensure that any incidents involving individuals with IDD are documented and reported according to regulatory requirements.
5. Health Education and Prevention:
- Provide education to individuals and their families on managing health conditions, medication administration, and preventive health measures.
- Offer guidance on healthy lifestyle choices, symptom management, and strategies for reducing the risk of health complications.
- Encourage and educate families on strategies for improving the individual’s quality of life and independence.
6. Quality Assurance and Safety:
- Ensure that all nursing care is delivered in accordance with best practices, individual safety protocols, and quality standards.
- Participate in regular case reviews and quality assurance initiatives to improve care delivery.
- Advocate for improvements in individual care and identify opportunities to enhance care quality and service delivery.
Qualifications:
Education: Bachelor’s Degree in Nursing (BSN or ADN) from an accredited nursing program.
Licensure: Valid Registered Nurse (RN) license in South Carolina.
Experience:
- At least 2 years of nursing experience, with a focus on home health or working with individuals with IDD preferred.
- Experience in case management, care coordination, or working in an interdisciplinary team is highly desirable.
Reports to: Regional RN Case Manager
Competency Requirements:
- Strong clinical skills and experience in performing nursing assessments and recertifications.
- Ability to develop and implement individualized care plans based on comprehensive individual assessments.
- Knowledge of IDD-related health conditions, treatment options, and individual care strategies.
- Strong communication skills to effectively interact with individuals, families, interdisciplinary teams, and external healthcare providers.
- Ability to work independently, manage a caseload, and prioritize tasks effectively.
- Cultural Competence: Sensitivity to and understanding of the diverse backgrounds and needs.
- Problem-Solving and Decision-Making: Strong analytical skills and the ability to make informed decisions based on data, individual needs, and organizational priorities.

Work Environment:
- The RN Case Manager will work in an office-based setting 08:30am-05:00pm Monday-Friday.
- The role requires travel to visit individuals in their homes, community settings, or healthcare facilities.
