Job Description Summary:
This is a full-time, day shift position at Mansfield hospital. Shift hours are M-F 8a-5p. No holidays. Must have BCOP certification or be able to obtain within 24 months of employment.
MINIMUM QUALIFICATIONS AND SPECIALIZED KNOWLEDGE
- BS or Doctor of Pharmacy (PharmD) degree from an accredited institution (Required)
- Current Ohio pharmacist license (Required)
- Board Certification in Oncology (BCOP) or eligible for examination and attainment within the first 24 months of employment (Required)
- BCOP certification must be maintained during length of employment in position (Required)
- Completion of a Pharmacy Practice residency or equivalent hospital experience (Required)
- Pharmacokinetic drug dosing experience
JOB SUMMARY
The Oncology Clinical Pharmacist has a focused approach in working with OhioHealth’s multi-disciplinary oncology care teams. Primary responsibilities include:
- Promoting evidence and value-based oncology & supportive medication therapy through close participation in the oncology medication therapy decision-making process with the multi-disciplinary care team.
- Recommending modifications and improvements in oncology & supportive medication therapy based on information obtained through patient interviews, objective monitoring parameters and the patient medical record.
- Modifying chemotherapy plans in the electronic health record (EHR), in collaboration with the multi-disciplinary team, to ensure plans are accurate for patient’s age and performance status, pre-medications are appropriate for patient-specific factors and relevant drug interactions have been identified and addressed.
- Communication and documentation of findings and pharmacist interventions with providers and providing appropriate alternatives to current treatment plans as needed.
- Position may include optimization of patient medication adherence through counseling, education, and application of motivational interviewing skills. Documentation of patient education at therapy initiation and follow-up, including review of oncology & supportive medication effectiveness, side effects, lab monitoring, expected outcomes of therapy.
- Understanding of Specialty Pharmacy Benefit Managers (PBMs) and formulary requirements. Identifying and recommending alternative medications for requests for non-formulary specialty drugs required by insurances.
- Providing experiential instruction to Doctor of Pharmacy Students and OhioHealth pharmacy residents.
Responsibilities And Duties:

50% - Reviews physician orders for correct drug choice, dosage form, dosing schedule, and possible drug-drug interactions or allergies. Contacts physicians to clarify orders as appropriate. Dispenses and compounds medications and dispenses drug therapy-related devices in accordance to standards of practice and OhioHealth pharmacy policies. Ensures all medications, chemicals and supplies are dispensed without suboptimal integrity. Ensures appropriate labeling for proper administration of medication. Interprets prescriptions and determines compatibility in accordance with standards of practice. Prepares care plan for patient to ensure optimal therapeutic outcomes and maximize compliance with therapy instructions.

20% - Manages individual's drug therapy by initiating, monitoring and modifying prescription issued to patient in a consulting relationship with a physician. Provides drug information to physicians, other health care professionals, and patients as needed.

10% - Counsels individuals with regard to their drug therapy by recommending drug therapy related devices and assisting in the selection of non-prescription drugs and appliances for the treatment of common diseases and injuries and providing instruction in their proper use.

10% - Assists with staff development and training. Provides educational programs for pharmacy, nursing, and medical staff as needed.

10% - Directs and guides pharmacy technicians and pharmacy support personnel.
