INTRODUCTION
Fairview is seeking a Family Medicine physician to join our primary care practice at M Health Fairview Clinic – Bass Lake. Located in the growing Maple Grove/Plymouth communities, M Health Fairview Clinic – Bass Lake provides comprehensive, patient-centered primary care for patients of all ages. The clinic offers a collaborative and supportive environment focused on whole-person care, with services including family medicine, pediatrics, onsite lab, and imaging for added convenience.
As part of continued growth and expansion in the northwest metro, the current Bass Lake clinic will transition to a new, larger clinic location with expanded services and increased capacity, anticipated to open in January 2027. In preparation for this exciting growth, we are actively recruiting additional physicians to support the expanding patient population and future care model.
ROLE AND RESPONSIBILITIES
- 9 provider primary care
- Patient encounters average 18-20 patients per day.
- 1 FTE = 34 patient contact hours per week.
- Call Frequency: approximately 1 per month - phone consultation only with first tier nurse triage.
- Openings: 1.0 FTE (Full Time) or less - Flexible schedule.
- As a Pioneer Accountable Care Organization (ACO), our physicians use a multidisciplinary team approach in managing clinical care.
- Fully integrated Electronic Medical Record (EPIC).
BASIC QUALIFICATIONS
- Graduate of an accredited ACGME/AOA Family Medicine residency training program.
- BC/BE with the ABMS or AOA.
- Licensed or ability to obtain licensure through the Minnesota Board of Medical Practice.
- Ability to obtain and/or maintain DEA certification for the State of Minnesota.
- Meet threshold criteria/qualifications for Credentialing and Privileges if applicable.

Conveniently located with free parking and easy access to surrounding northwest metro communities, Bass Lake offers providers the opportunity to practice in a well-established clinic serving a diverse and growing patient population in Maple Grove.

- $281,854 - $313,172 Annual based on full time (1.0 FTE)
