INTRODUCTION
The University Counseling Center (UCC) at the University of Utah enthusiastically welcomes applications for a 12-month full-time (40 hours/week) Care Coordinator to become an integral member of our dynamic multidisciplinary team.
ROLE AND RESPONSIBILITIES
Responsibilities include:
- Provision of face-to-face care management for students in need of longer term or more specialized treatment
- Coaching and skills-based training for students navigating care, campus and community referral follow-up
- Collaboration and outreach with campus partners who provide case management for University of Utah students
- Active collaboration with community mental health providers and agencies, including those who offer free and low-cost treatment
Responsibilities
- Provides case management for students requesting or are recommended for a transfer of care out of UCC services
- Provides case management for in-house referrals to internal UCC services
- Provides follow up case management for clients of concern as needed
- Serve as a bridge to appropriate campus and community resources; facilitate access and communication; monitor and ensure appropriate services are delivered
- Assist in coordination of patient discharge from inpatient hospitalization, coordinating with other campus-based case managers in the Dean of Students office and Student Health & Wellness
- Monitor and coordinate direct referrals between the Student Health Center and the University Counseling Center
- Provide coaching and skills training to student on self-advocacy, navigating health insurance, and connecting with mental health and other wellness services
- Provide outreach presentations to the campus community
- Processes requests for reassignments when established UCC clients have not been seen by assigned counselor in over 3 months or longer
- Develop and maintain comprehensive knowledge of community resources
- Rotate weekly consultation groups to meet with clinicians re: case management services and mutual clients
- Other relevant duties as assigned

MINIMUM QUALIFICATIONS
Bachelor’s degree in Health Care Administration, Business, Communication, Nursing or related field, or equivalency (one year of education can be substituted for two years of related work experience). Demonstrated human relation and effective communication skills are also required. Experience in oncology, multidisciplinary clinics, and/or working with high-acuity, complex patients may be preferred.
This position is patient-sensitive and must fulfill all associated requirements. We protect our patients, coworkers, and community by requiring all patient-sensitive employees to be immunized according to CDC standards and hospital policy. Limited exemptions may be made for documented medical contraindications or religious beliefs that object to vaccinations.

- Experience working with a disciplinary setting, with particular preference for those with experience working directly with college student mental health and wellbeing
