Join DaVita Integrated Kidney Care (IKC) as a Care Coordinator – Nurse Practitioner Primary Care Pilot
The Opportunity
It takes a village to care for our patients, and as a Care Coordinator (CC), you’ll be at the forefront of healthcare innovation. This is not your standard scheduling role; you will be a key player in a specialized program where Nurse Practitioners (NPs) serve as independent, "primary care-lite" providers for complex CKD and ESKD patients.
We are looking for a candidate who is excited about care redesign and thrives in an evolving environment. If you have a passion for nephrology, dialysis, or the future of primary care, you belong in our Village.
Location & Logistics
- Work from your home office with quarterly travel for team-building and nephrology office visits in Des Moines, IA and Lincoln, NE.
- Zone & Timing: Must be based in or aligned with Central Time (CST) to support our NE and IA markets.
- Schedule: Full-Time, Monday–Friday. Daily start time between 7:00–8:00 AM
- Home Office Requirements: Quiet, private space with high-speed internet.
Responsibilities
- You will be the primary scheduler and administrative partner for our NPs. You will ensure the patient journey is seamless from the moment of intake to the completion of the visit.
- Strategic Scheduling: Own the scheduling process for NP patient panels. You will balance complex schedules, facilitate outreach to patients for upcoming visits, and ensure the NP’s time is optimized for high-impact patient interactions.
- Care Coordination: Assist in navigating patients through the continuum of care. Help move patients through identified care pathways under the guidance of the NP and the local nephrology practice team.
- Communication Hub: Act as the primary liaison between patients, the NP, and the partner nephrology office. Manage inbound and outbound communications to resolve scheduling conflicts, confirm appointments, and collect patient information.
- Innovation Partner: Participate in workflow optimization. Refine processes and improve patient experience.
- Documentation & Compliance: Maintain accurate, real-time documentation in the Electronic Medical Record (EMR) to ensure care teams have the information they need to succeed.
- Resource Navigation: Identify and coordinate resources, referrals, and appointments, collaborating with the broader care team to ensure patients have access to the care they need.
Qualifications
- High School Diploma or GED required.
- Experience: 2+ years of experience in a professional office or clinical environment.
- Tech-Savvy: Proficiency in Microsoft Office (Excel, Outlook) and experience with Electronic Medical Records (EMR).
- The "Innovator" Mindset: You are excited about care redesign. You aren't just looking for a task-oriented role; you want to be part of a team building something new. You are comfortable with ambiguity and growth.
- Excellent Communication: You can communicate with clarity, empathy, and professionalism with patients, providers, and office staff.
- Autonomous & Collaborative: You are a self-starter who works well remotely, yet you value being a highly visible, communicative member of a small, agile team.
- Discretion: Ability to handle confidential patient information with the highest level of integrity.
Preferred Qualifications
- Passion for the Mission: A genuine interest in nephrology, dialysis, or primary care. You are excited about the challenge of improving health outcomes for CKD and ESKD patients.
- Clinical Office Experience: 1–3 years of experience in a medical office, particularly in scheduling, referrals, or care management.
- Clinical Exposure: CNA, MA, or similar clinical background is a major plus.
- One year of experience using Cerner (EMR).
Why You’ll Love Working Here:
- Make a direct impact on the lives of patients with complex chronic conditions.
- Be part of a mission-driven, collaborative care team.
- Flexible hybrid schedule with opportunity for professional growth.
What We’ll Provide:
More than just pay, our DaVita Rewards package connects teammates to what matters most. Teammates are eligible to begin receiving benefits on the first day of the month following or coinciding with one month of continuous employment. Below are some of our benefit offerings.
- Comprehensive benefits: Medical, dental, vision, 401(k) match, paid time off, PTO cash out.

- Professional development programs: DaVita offers a variety of programs to help strong performers grow within their career and also offers on-demand virtual leadership and development courses through DaVita’s online training platform StarLearning.

Job Roles at DaVita Companies
How to Get Visa Sponsorship in DaVita Visa Sponsorship USA
Target roles that align with DaVita's core clinical needs
DaVita's sponsorship activity is concentrated in healthcare and technical functions. Focus your applications on roles in nursing, biomedical engineering, data analytics, and clinical operations, where the company has a demonstrated history of sponsoring international talent.
Prioritize Green Card-track roles if long-term status is your goal
DaVita's sponsorship activity skews heavily toward permanent residence filings. If your goal is a Green Card and not just temporary work authorization, DaVita's hiring patterns make it a stronger long-term bet than many healthcare employers.
Engage DaVita's talent acquisition team early in your job search
Healthcare employers with active sponsorship programs often fill roles before they're widely listed. Reaching out to DaVita recruiters directly, especially for clinical and corporate positions, can surface opportunities that aren't visible through standard job searches.
Verify OPT and CPT eligibility before applying for entry-level roles
DaVita has sponsored F-1 OPT and CPT candidates, making it a viable option for recent graduates from U.S. programs. Confirm your OPT end date and STEM extension eligibility before applying so you can address timeline questions confidently during interviews.
Use Migrate Mate to find DaVita openings filtered by sponsorship history
Searching broadly wastes time on employers who won't sponsor. Migrate Mate surfaces verified sponsors so you can filter by real sponsorship history and go straight to DaVita's active openings rather than guessing at their hiring intent.

Does DaVita sponsor H-1B visas?
Yes, DaVita sponsors H-1B visas, though the company's overall sponsorship activity leans more heavily toward permanent residence pathways such as EB-2 and EB-3. H-1B sponsorship at DaVita tends to be concentrated in specialized clinical, technical, and corporate roles where the company has difficulty filling positions with U.S. workers alone. If you're pursuing an H-1B, focus on roles that require niche expertise.
What visa types does DaVita sponsor?
DaVita sponsors several visa categories including H-1B, TN, F-1 OPT, F-1 CPT, and permanent residence through the EB-2 and EB-3 employment-based Green Card pathways. The company's strongest sponsorship volume sits in the Green Card category, which reflects a pattern of investing in long-term retention of international employees rather than just short-term work authorization.
Which departments or roles at DaVita are most likely to receive visa sponsorship?
Sponsorship at DaVita is most common in clinical and patient care functions, biomedical and engineering roles, data and technology positions, and certain corporate functions tied to operations or compliance. Healthcare-specific credentials, such as nursing licenses or clinical certifications, strengthen your case considerably. Roles that require specialized knowledge and have a documented shortage of qualified U.S. applicants are where DaVita's sponsorship activity is most concentrated.
How do I find visa-sponsored job openings at DaVita?
The most efficient way is to use Migrate Mate, which lists DaVita's open positions alongside verified sponsorship data so you can confirm the company's track record before applying. From there, apply directly through DaVita's careers portal and mention your visa status upfront in your application or initial recruiter conversation. Being transparent about your sponsorship needs early prevents wasted time on both sides.
How do I approach the sponsorship conversation during DaVita's hiring process?
Raise your visa sponsorship needs during the initial recruiter screen, not after an offer is extended. DaVita's talent acquisition team is experienced with sponsorship cases, so a direct question about their ability to sponsor your specific visa category will get you a straight answer. Come prepared with your current status, OPT or authorized stay end date if applicable, and your preferred visa pathway. This signals that you understand the process and reduces friction for their immigration team.
