Responsibilities/Job Description:
Oversee the operation and management of assigned clinical pharmacy programs, IVIG Stewardship Program, Consulting Services for Infusion Financial Securing. Provide clinical care to patients of both programs, collaborate with the support staff and ensure adherence to all policies and procedures.
Responsibilities:
- Provide appropriate pharmaceutical care including patient counseling and prospective drug use evaluation to all patients receiving home IV therapy.
- Create, implement and manage a clinical program to address the growing payor concerns realized surrounding Immunoglobulin therapy administration in the Infusion Center setting.
- Create, implement and manage a clinical program for consultation with infusion finance securing teams reporting through Revenue Cycle. Be readily available for on demand prior authorization submission, letter of medical necessity questions and clinical review advocating for the care of all patients receiving intravenous therapy from all of the M Health Fairview Infusion Centers.
- Monitor the payor landscape and make adjustments to programs as needed depending on shifts and clinical changes that may affect payor coverage for past and future administrations.
- Be the managing pharmacist for the “RX Benefit Process” transitioning prescriptions from medical to pharmacy benefit when payors mandate shift within the M Health Fairview system, help maintain SOP and collaborate with interdisciplinary teams for any necessary process improvement changes needed.
- Act under a collaborative practice agreement with assigned providers under Cancer Care and Geneticists.
- Perform initial patient evaluation as it pertains to the prescribed therapies. Collect, analyze, interpret, and evaluate appropriate data relative to medication therapy. Obtain histories and apply these along with other patient data to optimize drug therapy management. Work with other healthcare providers to evaluate patients to determine appropriateness for home and infusion center IV therapy.
- Evaluate the significance of signs and symptoms relative to disease processes and makes recommendations for the modification of drug therapy.
- Evaluate the significance and clinical importance of adverse reactions and complications associated with drug therapy. Adjust administration of medication and consult with patient’s provider if orders need to be modified for safe medication administration.
- Monitor patient records for potential drug/drug or drug/food interactions as well as patient allergies.
- Provide counseling and answer questions from patients, family and health care professionals concerning drug therapy.
- Provide prescription drug therapy to patients/customers ensuring accurate labeling and dispensing of medications in a timely fashion.
- Become a content expert for Home Care, Infusion Center and Pharmacy billing process through both the medical and pharmacy benefits.
- Evaluate patient supply of medications and IV access supplies.
- Assist patient and other healthcare providers on maintaining IV line for administration of medication.
- Monitor and evaluate clinical outcomes for future data reporting, education and publication.
- Maintain area of expertise for the M Health Fairview system in Immunoglobulin Therapy and Rare Disease Therapy in the skills and theory of the practice of pharmacy by reviewing literature, attending in-services, seminars and conferences.
- Ensure dispensing of prescriptions in accordance with all legal requirements, regulatory agency Infusion center, Retail and Fairview Home Infusion Policy and Procedures.
- Oversee the onboarding schedule and training of the Rare Disease Clinical Pharmacist. Act as primary contact for the Rare Disease Clinical Pharmacist for clinical concerns, and questions.
- Act as the primary preceptor for any learners (students and residents) on rotation with the Medication Access Pharmacist.
