How to Get Visa Sponsorship in Physical Therapist
Nonprofit Hospital Systems File Cap-Exempt H-1B Year-Round
Mayo Clinic, Johns Hopkins, Cleveland Clinic, Mass General Brigham, and university-affiliated hospitals sponsor PTs on cap-exempt H-1B with no lottery and no seasonal filing restriction. These systems operate dedicated international PT recruitment programs with relocation support.
EB-3 Professional Green Card Is the Most Direct Pathway
Large hospital systems and rehabilitation companies sponsor PTs for EB-3 professional immigrant visas through PERM labor certification leading directly to permanent residency. HCA Healthcare, Encompass Health, and Select Medical file hundreds of PT petitions annually.
FCCPT and NPTE Must Be Completed Before Visa Filing
Foreign-educated PTs must complete FCCPT credential evaluation and pass the National Physical Therapy Examination before any employer can file a visa petition. State licensure requirements vary and some states require additional jurisprudence exams.
Orthopedic and Neurological Specializations Are in Highest Demand
PTs with orthopedic manual therapy, stroke rehabilitation, spinal cord injury, and sports medicine expertise fill the most acute clinical gaps. Board certification as OCS, NCS, or SCS through ABPTS increases compensation by 8 to 15%.
Travel Therapy Agencies Sponsor for Contract-to-Permanent Placements
CompHealth, Jackson Therapy Partners, and MedPro International recruit international PTs for contract positions that convert to permanent hospital placements. These agencies handle visa processing, credential evaluation, and NPTE preparation as part of recruitment.
Rural and Underserved Areas Offer the Fastest Sponsorship Timelines
PT shortages are most severe in rural communities and medically underserved areas where clinics operate with months-long patient waitlists. Employers in these areas sponsor urgently and often include signing bonuses of $5,000 to $20,000.
