Understand the J-1 vs. H-1B pathway before you apply
Most international medical graduates enter through J-1 exchange visitor programs during residency, then transition to H-1B after training. Knowing which visa you currently hold determines your waiver options and how quickly you can work independently after graduation.
Target Conrad 30 waiver positions strategically
Each state allocates 30 Conrad waivers annually for J-1 physicians willing to work in underserved areas. These positions are competitive and fill quickly, so applying early in the waiver cycle, typically starting in late summer, significantly improves your chances of securing one.
Confirm your medical degree meets ECFMG certification requirements
International medical graduates must hold ECFMG certification before entering U.S. residency programs or obtaining H-1B sponsorship for clinical roles. Employers verify this alongside your medical license, so completing certification before job searching removes a common early-stage hiring obstacle.
Prioritize employers with a documented history of physician sponsorship
Large hospital systems, academic medical centers, and federally qualified health centers sponsor international physicians far more consistently than private practices. Filtering your search to these employer types reduces the risk of withdrawals after an offer is extended.
Factor in state medical licensing timelines when negotiating start dates
State medical licenses can take three to six months to process, and H-1B petitions cannot be approved without one in most clinical contexts. Build this timeline into your offer negotiation so your employer files the H-1B petition before the license becomes the bottleneck.
Use Migrate Mate to find physicians roles with confirmed sponsorship
Migrate Mate lists physician and medical doctor positions from employers already willing to sponsor. Browsing by role and visa type lets you focus your applications on organizations with active sponsorship programs rather than spending time on positions that exclude international candidates.

Find Medical Doctor JobsFrequently Asked Questions
What visas are commonly used to sponsor medical doctors in the U.S.?
H-1B and J-1 are the two primary visa types for international physicians. J-1 is used during residency and fellowship through ECFMG-sponsored programs. H-1B is typically used for attending physician roles after training. EB-2 and EB-3 immigrant visas are common pathways for doctors pursuing permanent residence, particularly through PERM labor certification.
Do I need a U.S. medical license to get H-1B sponsorship as a physician?
Yes, in almost all cases. USCIS requires evidence that you can lawfully practice medicine in the state where you'll work, which means a valid state medical license is effectively a prerequisite for H-1B approval in clinical roles. Some employers will extend a conditional offer and begin the H-1B process while your license application is pending, but the petition cannot be approved without the license confirmed.
What is a Conrad 30 waiver and how does it affect my visa status?
The Conrad 30 program allows J-1 physicians subject to the two-year home residency requirement to obtain a waiver by committing to work in a medically underserved area for at least three years. Once the waiver is approved, your employer can file an H-1B petition on your behalf, converting your status from J-1 to H-1B and letting you remain in the U.S. without serving the home residency requirement.
How competitive is H-1B sponsorship for physicians compared to other professions?
Physicians have a meaningful advantage in the H-1B process. Many physician roles qualify for cap-exempt H-1B petitions when filed through nonprofit hospitals, academic medical centers, or federally qualified health centers, meaning they bypass the annual lottery entirely. This makes the visa pathway far more predictable for doctors than for most other sponsored professions, where lottery selection is the primary obstacle.
Where can I find medical doctor jobs that offer visa sponsorship?
Migrate Mate lists physician roles from employers with confirmed sponsorship programs, filterable by visa type and specialty. This is more efficient than searching general job boards, where sponsorship availability is often unclear or misrepresented. Focusing on employers who have previously filed H-1B or J-1 petitions for physicians is the most reliable indicator that they have an established process for international hires.
What is the prevailing wage requirement for sponsored Medical Doctor jobs?
U.S. employers sponsoring a visa must pay at least the prevailing wage, which is what workers in the same role, area, and experience level typically earn. The Department of Labor sets this rate to make sure companies aren't hiring foreign workers simply because they'd accept lower pay than a U.S. worker. It varies by job title, location, and experience. You can look up current prevailing wage rates for any occupation and location using the OFLC Wage Search page.
