How to Get Visa Sponsorship in Healthcare
Identify which visa pathway fits your specific healthcare role
Healthcare visa sponsorship varies dramatically by occupation. Physicians often use J-1 waivers, nurses may benefit from Schedule A green card processing, and allied health professionals typically pursue H-1B petitions. Research the pathway that applies to your exact role before investing time in applications.
Start your U.S. credential evaluation early
Most healthcare roles require credential verification through organizations like CGFNS, ECFMG (for physicians), or state licensing boards. These evaluations can take months to complete and often involve document authentication from your home country. Beginning the process as early as possible prevents delays when an employer is ready to sponsor.
Target employers in Health Professional Shortage Areas
HPSAs face persistent staffing challenges, which makes employers in these areas more willing to sponsor international healthcare workers. Federally qualified health centers, rural hospitals, and community clinics in HPSAs often have experience navigating the sponsorship process. Working in a shortage area can also unlock additional immigration benefits like J-1 waivers.
Get your English proficiency scores early
Many healthcare licensing processes and the VisaScreen certification require English proficiency exams like TOEFL iBT or IELTS. Scores are often valid for only two years, so time your test to align with your job search and licensing timeline. Having current scores ready eliminates a common bottleneck in the sponsorship process.
Consider health informatics as a bridge role
Health informatics positions combine clinical knowledge with data analysis and IT skills, and they often qualify for both H-1B sponsorship and STEM OPT. These roles at hospital systems, EHR companies like Epic and Cerner, and health tech startups value international candidates with clinical backgrounds. It can be a strategic entry point into the U.S. healthcare system while you pursue clinical licensure.

What visa options are available for healthcare workers in the U.S.?
The main pathways depend on your specific role. H-1B visas cover specialty occupations like physicians, pharmacists, and therapists that require at least a bachelor's degree. J-1 visas are common for medical residencies and exchange programs, with waiver options for physicians who serve in underserved areas. Registered nurses and physical therapists may benefit from Schedule A designation, which streamlines the green card labor certification process.
What is VisaScreen, and do I need it?
VisaScreen is a credentials verification program administered by CGFNS (Commission on Graduates of Foreign Nursing Schools) that is required for certain healthcare professionals seeking U.S. work visas or green cards. It applies to nurses, physical therapists, occupational therapists, medical technologists, and several other clinical occupations. The process includes a credentials review, English language proficiency test, and verification of your license. You should check whether your specific occupation is on the VisaScreen-required list before applying for jobs.
Do healthcare employers in shortage areas sponsor visas more readily?
Health Professional Shortage Areas (HPSAs) designated by HRSA often have employers who are more willing to sponsor because they struggle to fill positions with domestic candidates. Rural hospitals, federally qualified health centers (FQHCs), and community health organizations in these areas frequently use visa sponsorship as a recruitment tool. Working in an HPSA can also open additional pathways like J-1 waiver programs for physicians.
Can I work in healthcare administration with visa sponsorship?
Yes. Healthcare administration roles - such as hospital operations managers, health informatics specialists, and healthcare consultants - can qualify for H-1B sponsorship when they require a bachelor's or master's degree. These positions are often easier to sponsor than clinical roles because they do not require U.S. clinical licensure. Large hospital systems like HCA Healthcare, Kaiser Permanente, and CommonSpirit Health hire for these positions regularly.
What is the prevailing wage requirement for sponsored Healthcare jobs?
When a U.S. employer sponsors a foreign worker for a work visa, they are legally required to pay at least the "prevailing wage" — the average wage paid to workers in the same occupation, in the same geographic area, with similar experience. This is set by the Department of Labor to prevent employers from hiring foreign workers at below-market rates. The prevailing wage varies significantly by role, location, and experience level — for example, a healthcare in Texas will have a different prevailing wage than the same role in a smaller state. You can look up current prevailing wage rates for any occupation and location using the OFLC Wage Search.
