Cap-Exempt H-1B at Nonprofit Hospitals Files Year-Round
Mayo Clinic, Cleveland Clinic, Johns Hopkins, Mass General Brigham, and hundreds of nonprofit hospital systems file cap-exempt H-1B for physicians with no lottery and no annual limit. These positions start during or immediately after residency completion.
J-1 Conrad 30 Waiver Places Physicians in Underserved Areas
Each state sponsors up to 30 J-1 waiver slots annually for physicians who commit to three years of service in a Health Professional Shortage Area. Primary care, psychiatry, and OB/GYN in rural and inner-city locations fill these waivers fastest.
Primary Care and Psychiatry Have the Shortest Sponsorship Timelines
Internal medicine, family medicine, and psychiatry positions in shortage areas receive sponsorship offers during residency with signing bonuses of $20,000 to $50,000. These specialties have the highest employer urgency and lowest domestic recruitment competition.
ECFMG Certification Is Required Before Any Visa Filing
International medical graduates must hold ECFMG certification, which requires passing USMLE Steps 1 and 2 CK plus meeting medical school credential requirements. Complete this before applying to residency programs or employer-sponsored positions.
Academic Medicine Combines Research With Cap-Exempt Sponsorship
Physician-scientists at university medical centers conducting NIH-funded research qualify for cap-exempt H-1B and O-1 extraordinary ability based on publications and grants. Academic positions pay $200,000 to $350,000 depending on specialty and research funding.
EB-1B Outstanding Researcher and National Interest Waiver Accelerate Green Cards
